By Qamar S, 15 years in US medical billing · Last reviewed October 11, 2026

The official 2027 Medicare Part B premium hasn’t been announced yet. The 2026 Medicare Trustees Report projects it at about $209.50 a month, up roughly $6.60 from the 2026 premium of $202.90. CMS usually announces the real number in November.

We’ll update this page the day CMS announces the 2027 premium, deductible and IRMAA brackets. Until then, treat every 2027 number you see online as a projection.

How the premium affects your Social Security check

If you get Social Security, your Part B premium is taken out of your monthly payment. The 2027 cost-of-living adjustment (COLA) and the new Part B premium both take effect in January, so the change in your deposit is your COLA increase minus any Part B increase.

The hold harmless rule protects most people: if the Part B increase would make your Social Security check go down, your premium increase is limited to the amount of your COLA. It doesn’t apply if you pay IRMAA, are new to Medicare, or don’t have premiums deducted from Social Security.

IRMAA: will you pay more?

Higher earners pay an income-related monthly adjustment amount (IRMAA) on top of the standard premium. For 2027, Medicare will use the income on your 2025 tax return. See IRMAA brackets for 2027 for the 2026 tiers and how to appeal with Form SSA-44 if your income dropped because of retirement or another life-changing event.

What else to watch for 2027

From 15 years in billing: if you pay Part B premiums directly (not through Social Security), set a reminder to check your first bill in 2027. I’ve seen premiums go unpaid for months simply because people kept paying the old amount, and Medicare can end coverage for unpaid premiums.
