Health Plan Cost Comparison Calculator (2027 Open Enrollment)

By Qamar S, 15 years in US medical billing · Last reviewed September 26, 2026

The cheapest premium isn’t always the cheapest plan. This calculator adds a year of premiums to what you’d pay for care in a light year, a typical year and a worst-case year, so you can compare up to 3 plans by their real total cost. Open Enrollment for 2027 coverage starts November 1.

Compare health plans by total yearly cost

Enter up to 3 plans from HealthCare.gov, your state marketplace or your employer. Use the plan’s numbers for in-network care.

How to use it

  1. Copy each plan’s monthly premium, deductible and out-of-pocket maximum from HealthCare.gov, your state marketplace or your employer’s plan documents.
  2. Estimate a light year and a typical year of care, using allowed amounts (not list prices).
  3. Compare the totals. The highlighted plan is cheapest in each row.

From 15 years in billing: look hard at the worst-case row. Most people pick on premium alone, then a single ER visit or surgery shows them the plan’s real cost. If you couldn’t cover a plan’s out-of-pocket maximum, it’s probably not your plan.

Before you choose, check that your doctors are in network (PPO vs HMO), learn how metal tiers work and read our Open Enrollment 2027 guide.
